12 On Health: Blood pressure program
Doctors in New York are treating patients who have high blood pressure with a new program. 
The 'Treat to Target' program is based on two key principles that aim to provide patients with the tools they need to better control their blood pressure.
Officials at NYC Health and hospitals say the program first empowers nurses by allowing them to see their patients more often between doctor's visits so they can better monitor the treatment plan and have a more in-depth understanding of their patients' progress.
The program also allows the sharing of more information among nurses and doctors so they can reach out to patients that don't have their blood pressure under control. 
Officials say in the last year, 72,000 patients across the city had their blood pressure controlled and that's 2,000 more than the year before.
Doctors say the program is free to participate in and that patients shouldn't view it as part of their routine care.
