The Community Partnership Program run by the Nassau County District Attorney's Office is giving former convicts a second chance.

Located in the heart of Hempstead, CPP serves as a one-stop shop that provides social services to individuals re-entering the population after serving time behind bars, as well as others who are struggling.

It offers mental health and substance abuse help, vocational skills and job preparedness. It also has an emergency food pantry and runs gang intervention programs in schools.

Andrea Johnson struggled with a 25-year addiction to heroin and says the program saved her life. The 64-year-old grandmother says it helped her beat her addiction, get her back on her feet and get a job. She now works as a case manager for CPP.
