The National Transportation Safety Board (NTSB) says that driver fatigue is among several factors that contributed to a deadly tour bus crash on I-95 in The Bronx last year.

In a final report released Tuesday, the board said that the older model Coach bus didn't have seatbelts installed, that it was traveling above the speed limit at 78 mph and that the guard rail it hit was not equipped to handle a vehicle of its size.

The bus was heading back from a casino trip in Connecticut when it struck a metal divider and flipped on its side, killing 15 people. The driver, Ophadell Williams, of Brooklyn, faces multiple vehicular manslaughter charges and up to 15 years in prison if convicted.

The crash has also led to a federal crackdown on the safety of bus companies.
