Findings released from MTA audit prompted by 2024 subway explosion

The MTA inspector general on Thursday released the results of an audit that found deficiencies in NYC Transit's substation maintenance program.

A 2024 transformer explosion at the State Street substation in Brooklyn prompted the report, which stranded more than 4,000 riders for two hours.

The audit revealed that 19% of required maintenance checks were not completed that year. It also found that at State Street, crews missed 12 of 23 assignments before the blast.

The audit also said that fire alarm warnings were ignored and some workers were not wearing required protective gear.
