A new care management facility with a focus on various chronic conditions was unveiled with a special ribbon-cutting ceremony in The Bronx Tuesday.
Health Home on Townsend Avenue is devoted to helping people with HIV, AIDS and substance abuse issues.
Officials say the goal is to help them live healthier, happier and more productive lives.
