Report released investigating 1 train derailment from last month

Approximately 22 passengers and three MTA crew members suffered minor injuries in the incident.

The National Transportation Safety Board released its preliminary report of the derailment of a 1 train that took place earlier this month.

The northbound 1 train collided with another train near the 96th Street station on Jan. 4. Investigators say the flagger directing the train that was headed for maintenance lost radio communications with the system supervisor operating the controls. 

That supervisor did not receive the flagger's instructions to stop, causing the collision to take place. Approximately 22 passengers and three MTA crew members suffered minor injuries in the incident.

The NTSB will now focus on the operations of NYC Transit.
